Ellsworth Elementary Middle School students in grades PreK through 3rd will now have the opportunity to receive dental care—both cleanings and examinations—right on campus during school hours. We are excited to offer this service this April—sign up now!
Sweet Smiles will be coming to Ellsworth Elementary School this April to perform dental cleanings and examinations. This is a convenient and easy way to help your student maintain a healthy smile without having to schedule off-campus appointments.
For Families with Students in Grades 4-8: Please note that this current opportunity is for students in grades PreK-3 only. More communication will be coming about opportunities in the fall as we hope to expand this opportunity to all students at EEMS.
How to Sign Up: If you are interested in this service for your child, simply fill out the attached form and return it to the EEMS front office or school nurse before February 12, 2026.
A hard-copy of the form can be found in the main office, or can be sent home with your student upon request. Most dental insurance plans are accepted, and there is no copay required. If you do not have insurance or prefer to pay out-of-pocket, self-pay options are also available.
